What this policy covers

This policy describes what happens to information you submit through this website — the appointment request form and the voice assistant, if you use it. It does not describe how The Connection Chiropractic handles your medical records or protected health information inside the clinic. Ask the front desk for the clinic's Notice of Privacy Practices, which is the document that covers that.

What we collect

When you submit the appointment form we collect the name and phone number you enter, and — if you choose to provide them — your email address, the reason you are seeking care, your preferred callback time and anything you write in the notes field.

We also record which page the form was submitted from, so we know how you found us.

Please do not send detailed medical history, diagnoses or anything else sensitive through this form. It is an appointment request, not a medical record. Bring that information to your visit.

Why we collect it

To call you back and book your appointment. That is the only reason. We do not sell your information, we do not rent it, and we do not share it with advertisers or data brokers.

Who can see it

Submissions are stored by Netlify, the company that hosts this website, and are emailed to the clinic. Only clinic staff read them.

If you talk to the voice assistant

This site may offer an AI voice assistant that can answer questions about hours, pricing and services, and can take your name and phone number to pass on to the clinic as an appointment request. Those conversations are processed by ElevenLabs, our voice technology provider.

The assistant is not a clinician. It cannot diagnose you, it cannot give you medical advice, and nothing it says is a substitute for seeing Dr. Tyler. If you have a medical question, call the clinic and speak to a person.

Cookies and tracking

This website does not set advertising cookies and does not run third-party analytics or tracking pixels. There is no cookie banner because there is nothing to consent to.

How long we keep it

Appointment requests are kept as long as they are useful for booking and following up with you, and are deleted after that. If you would like your submission deleted sooner, ask us and we will do it.

Links to other sites

This site links out to our supplement shop, our sister business The Main Street Spa, and our online booking system. Once you follow one of those links you are on somebody else’s website, under their privacy policy, not ours.

Changes

We will update this page if what we do changes. The date at the top tells you when it was last revised.
